Based on observation the licensee did not comply with the section cited above when a bottle of bleach and a spray bottle of Lysol that contained liquid confirmed by smell, located in the children’s designated bathroom that was accessible to all children in care. LPAs also observed a large can of Lysol disinfectant spray in the children’s sleeping room that was accessible to all children which posed an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately relocated the chemicals to an area that was inaccessible to the children in care.

Based on observation, interview and record review, the licensee did not comply with the section cited above when two (2) infants did not have any record or even an attempt to document 15 minute checks required be by safe sleep regulations over a period of one year which poses an immediate health, safety, or personal rights risk to persons in care.

Resolution: Licensee will conduct 15 minute checks and will provide proof that she or her staff are indeed following Safe Sleep Regulations. By the required date via photos or facility visit by LPA

Based on observation, the licensee did not comply with the section cited above in having a baby walker located in the children’s sleeping room which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately relocated the baby walker to an area that was inaccessible to the children in care.

Based on observation the licensee did not comply with the section cited above when LPAs observed additional unserviceable sleep mats in the play pen. Upon further inspection, LPA's observed a white dry substance that appeared to be milk under the mattress indicating the area has not been cleaned for some time which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall sanitize the play pens and provide new serviceable sleeping mats with clean sheets. LPA will return at an unannounced time to confirm.

Based on observation the licensee did not comply with the section cited above in LPA’s finding a small toy finger capable of being swallowed by a child in care while in the play pen which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately removed the item.
