Based on record review, the licensee did not comply with the section cited above. Staff member does not have evidence of a current tuberculosis clearance, not more than one year prior to or seven days after initial presence which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have staff member obtain evidence of a current tuberculosis clearance and submit copy to Fresno Community Care Licensing by 07/28/2023.

Based on record review, the licensee did not comply with the section cited above. Several children's files were missing immunization records which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will request immunization records for the children and submit copies to Fresno Community Care Licensing by 08/05/2022.

Based on record review, the licensee did not comply with the section cited above. Based on observation, the licensee did not comply with the section cited above. Licensee had multiple items such as hand sanitizers, lotions, and other products throughout the areas accessible to children which poses a potential health, safety or personal rights risk to persons in care.

Resolution: During today's inspection, Licensee removed all items and placed them in an area inaccessible to children.

Based on record review, the licensee did not comply with the section cited above. Licensee has an infant in care and does not have an Individual Infant Sleeping Plan [LIC 9227 (3/20)] completed which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have an Individual Infant Sleeping Plan [LIC 9227 (3/20)] completed for each infant up to 12 months of age the provider has in care and included in the infant's file at the facility and submitted to Fresno Community Care Licensing by 08/05/2022.

Based on record review, the licensee did not comply with the section cited above. Infant's safe sleep log was last completed in May 2022. Licensee could not provide current documentation of safe sleep log which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will physically check on sleeping infants every 15 minutes and keep documentation on safe sleep log. Licensee will submit copy of safe sleep log showing two weeks of documentation to Fresno Community Care Licensing by 08/05/2022.

Based on record review, the licensee did not comply with the section cited above. Children's files were missing signed LIC 995a which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will request child's parent or authorized representative to sign and date the bottom portion of the notice form LIC 995A (8/06) and submit copies to Fresno Community Care Licensing by 08/05/2022.

Based on record review, the licensee did not comply with the section cited above. Licensee does not have immunization records on file for staff member that verifies immunization against measles which poses a potential health, safety or personal rights risk to persons in care.
