Based on records review, the licensee did not comply with the section cited above. 3 out of 3 children's records had incomplete or missing documents (LIC 700, LIC 627, Infant Sleep Log) which poses a potential health, safety or personal rights risk to persons in care. LPA advised Licensee to have all documents corrrected and submit proof of correction to the Department for verification
Resolution: Licensee will have all documents completed/updated and will email copies of documents to LPA.
Based on observation, the licensee did not comply with the section cited above (see LIC809 for further details). This poses a potential risk to the health, safety, or personal rights of children in care.
Resolution: Licensee stated she will thoroughly clean and remove clutter from the dining room and kitchen area. A return inspection will be made by LPA after 12/15/22 to ensure the licensee has followed the above measures to clear this deficiency.
