Based on observation and interview, the licensee did not comply with the section cited above. While touring the facility, LPA observed Staff 2 escorting preschool children to the restrooms.The boys went into the boy’s restroom and the girls went into the girl’s restroom. Both restrooms are next to each other. Staff 2 came out of the boy’s restroom and girls were in the restrooms with no staff supervision, which poses a potential health, safety or personal rights risk to persons in care.
