Based on observation,interview and record review, the licensee did not comply with the section cited above when Adult #1 was observed providing care and supervision to children in care. This individual was neither cleared through the required background check process nor associated with any other licensed facility.
Resolution: Adult #2 will undergo a background check in accordance with Community Care Licensing Division requirements by the above date.
Based on observation, the licensee did not comply with the section cited above when LPA observed a total of thirteen (13) children present in care, four (4) of the present children were infants. This exceeds the maximum allowable capacity for the facility based on license specifications. which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: The licensee agreed to review the California Code of Regulations (CCR) Ratio and Capacity requirements applicable to their facility. Upon completion of the review, the licensee will sign the bottom of the document acknowledging their understanding of the regulatory standards.
Based on record review, the licensee did not comply with the section cited above when facility records revealed that both the licensee and her assistant only possessed expired CPR certifications at the time of inspection which poses a potential health, safety or personal rights risk to persons in care as this is a repeat violation.
Resolution: Licensee and her assistant will be recertified by the above date.
Based on record review, the licensee did not comply with the section cited above. Staff member does not have evidence of a current tuberculosis clearance, not more than one year prior to or seven days after initial presence which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have staff member obtain evidence of a current tuberculosis clearance and submit copy to Fresno Community Care Licensing by 02/17/2023.
Based on interview and record review, the licensee did not comply with the section cited. Licensee was unable to provide current documentation that providers are physically checking on the infant every 15 minutes. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will physically check on sleeping infants every 15 minutes and keep documentation on safe sleep log. Licensee will submit copy of safe sleep log showing two weeks of documentation to Fresno Community Care Licensing by 02/27/2023.
Based on record review, the licensee did not comply with the section cited above. Licensee currently owes a total of $630.00 in annual and late fees which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee was advised that she is past due on payment of licensing fees. Licensee will send a copy of the online receipt showing payment or a copy of the check that was sent to the department for payment. A copy of the receipt shall be sent to the Fresno Community Care Licensing by 02/28/2023.

Resolution: Licensee will complete mandated reporter training via www.mandatedreporterca.com and submit copy of certificate of completion to Fresno Community Care Licensing by 02/27/2023.
Based on record review, the licensee did not comply with the section cited above. Staff #2 does not have immunization records on file to verify immunity against influenza, pertussis, and measles which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide copy of immunization record for staff #2 to the Fresno Community Care office by 03/09/2023.
