Resolution: Per licensee, they will provide child #1 file to LPA by POC due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in one (1) out of two (2) staff files had an expired mandated reporter on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, staff #2 will complete mandated reporter training and provide proof of completion by POC due date.

Resolution: Per licensee staff 4 will provide missing immunization records by POC due date.

Resolution: Per licensee will have file for child #1 available for review by POC due date.

Based on [(observation), the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal righfacility has not conducted fire drill every six months which poses a potential risk to persons in care.

Based on [(observation) , the licensee did not comply with the section cited above, fire extinguisher was not serviced yearly which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will service fire extinguisher and submit verification by POC due date

Based on record review, the licensee did not comply with the section cited above in three out of three staff have expired mandated reporter training certificates which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Both licensees and assistant will provide copies of mandated reporter training certificates by POC due date.

Based on (record review), the licensee did not comply with the section cited above in three out of three children's files (child #1, child #2 and child #3 were not available for review, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide verification of children's file by POC due date.

Based on observation and interview, the licensee did not comply with the section cited above in which licensee has not maintained a 15 minute sleep log for infants in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit verification of sleep log for infants in care by POC due date.

Based on (observation) , the licensee did not comply with the section cited above,the valve on the required 2A 10BC fire extinguisher indicates fully charged with a service tag dated 01/04/2021 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will purchase 2A 10BC fire extinguisher and submit verification by POC due date.
