Based on observation, interview, record review, the licensee did not comply with the section cited above in Mandated Reporter training expired on 05/11/2025 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: per Licensee, she will conduct mandated reporter training and provide proof of completion to LPA by POC due date of 02/19/26.
Based on LPA observation, the licensee did not comply with the section cited above,licensing staff observed an infant propped-up on a pillow with a plushy ball and a bottle. This is an immediate health, safety or personal rights risk to children in care.
Resolution: Licensee removed all articles from the inside of the play yard.

Resolution: Licensee moved kitchen knives and raid making them inaccessible to children.
