Based on records reviewed, 4 out of 4 files did not have the mandated reporter certificate. Licensee's and Staff's mandated reporter certificate expired on 09/19/2023, and Staff 3 and Staff 4 have not completed the mandated reporter certificate, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete Mandated Reporter Certificate and will submit certificates to CCL by 02/09/2026 www.mandatedreporterca.com

Based on records reviewed, 4 out of 7 files did not have required documents, were missing signatures or missing forms. LPA reviewed with Licensee all required forms and missing forms in each file.

Resolution: Licensee stated that she will communicate with all parents and obtain all required documents for all children in care and will send pictures of the documents to LPA via text message or email by 02/05/2025.

Based on record review, the licensee did not comply with the section cited above in 4 out of 7 children files were missing the Consent for Emergency Medical Treatment (LIC627) forms, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit signed LIC627 forms to the Department by 02/05/2025.

Based on observation of children's records, the licensee did not comply with the section cited above. Based on file review, 2 infants out of 3 infants did not have Safe Sleep 15-minute checks logs, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided template of Safe Sleep Log (15 -minute checks). Licensee start logging 15 minute checks for each infant in care and maintain the information on file. Licensee will submit logs to the Department by 02/05/2025.
