Based on observation, the licensee did not comply with the section cited above in that there was lysol spray and bathroom cleaning spray under the cabinets in the kitchen and bathroom with no child proof locks which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee removed both items and placed them in high drawers while LPA L. Habtom was present. LPA L. Habtom reminded licensee to either place child proof locks or to check cabinets to be sure all hazadorous items are placed out of reach of children.
Based on record review], the licensee did not comply with the section cited above in that sleep checks are conducted but not documented for the infant enrolled which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LPA L. Habtom reminded licensee to document when checking slleeping infant every 15 minutes and to email to LPA L. Habtom by POC date.
Based on record review, the licensee did not comply with the section cited above in that one infant does not have proof of immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LPA L. Habtom asked that licensee email a copy of child's immunizations by POC date.
Based on record review, the licensee did not comply with the section cited above with providing proof of TDAP & MMR which poses a potential health, safety or personal rights risk to persons in care.
