Resolution: Licensee agreed to submit proof of her immunization record for MMR and the Flu to Community Care Licensing Office by 07/29/2022. Licensee also agreed to submit proof of Staff #2 immunization record for MMR & TDaP to Community Care Licensing Fresno Office by 07/29/2022.
