Based on observation, the licensee did not comply with the section cited above, Infant 1 2 and 3 were napping in a Fisher Price rock and play equipment and Infant 3's body was curled up which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to use the available play pen/yard in the FCCH by tomorrow, 6/17/2022 Licensee disposed the Rock and play equipment and will no longer be in use at the day care. Licensee shall submit the written plan of correction to CCL no later than 6/17/2022 on how to ensure that children's personal rights are not violated.
Based on observation, the licensee did not comply with the section cited above when Licensee woke up Child # 9 from nap which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit a written plan of correction to CCL no later than 6/17/2022 to ensure that children's personal rights are not violated while at the Family Child Care Home.
Based on observation, the licensee did not comply with the section cited above the Fisher Price Rock and Play Sleepers used by the infants during nap time were recalled by United States Consumer Product and Safety Commision on 4/12/2019 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: During the inspection, Licensee disposed off the 3 Rock and Play Sleepers. Licensee shall submit a write plan of correction to ensure that FCCH shall provide safe toys and play equipment in the FCCH.
Based on record review, the licensee did not comply with the section cited above Children (C) C1, C7 and C10 have no immunization record on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit proof of correction no later than 6/27/2022 and shall ensure that prior to admission, Licensee will obtain copy of the immunization record from the authorized representative.
Based on record review, the licensee did not comply with the section cited above, Licensee was not documenting the children's immunization (C2-C6, C8, C9) which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to document the childrens imunization which includes updating of PM 286 and shall submit written plan of correction to CCL no later than 6/27/2022
Based on record review, the licensee did not comply with the section cited above, Infant # 1 does not have LIC 9227, Individual Safe Sleep Plan on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to complete the LIC 9227 with the authorized representative of Infant # 1 and shall submit written plan of correction to CCL no later than 6/27/2022
