Based on observation, the licensee did not comply with the section cited above in that rat droppings were observed in an unlocked cabinet in the entertainment unit in the living room, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit proof of the cabinet mentioned above to be clean and free of defects or conditions which might endanger a child. Licensee will also submit a written plan on how this violation will not reoccur. Licensee will make these submissions no later than 5/7/25.
Based on interview and record review, the licensee did not comply with the section cited above in that the last drill was conducted 8/16/2022 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to conduct and doucment an emergency drill by 04/24/2023 and send proof via email to francisca.velazquez@dss.ca.gov by 04/24/2023.
Based on interview and record review, the licensee did not comply with the section cited above in that mandate reporter certifacte expired 11/12/2022 and it has not been updated which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee's agress to update mandated reporter certificate and submit proof by 04/24/2023 via email to francisca.velazquez@dss.ca.gov
