Based on interview and record review, the licensee did not comply with the section cited above in 3 of 10 Children Files, Child #7, 9-10, were not available for file review during inspection, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, they will find locate the files of Child #7, 9, & 10, and send them to LPA by POC date. Child #4 immunizations will be submitted as well.
Based on interview and record review, the licensee did not comply with the section cited above 1 of 10 files, Child #2, did not have a completed Individual Infant Sleeping Plan (LIC9227), which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, they will have the parent of CHild #2 complete the LIC9227 Individual Infant Safe Sleep form, and submit proof to LPA by POC date.
Based on LPA observation, the licensee did not comply with the section cited above. The fire extinguisher was observed to be fully charged, however, it has not been serviced since 2019 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states he will have the fire extinguisher serviced and send LPA a photo of the receipt and/or service tag.
Based on record review, the licensee did not comply with the section cited above. It was observed that the Licensee's and the assistat't certificates expired in 02/23, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states he and all assistant's will take the training and send LPA copies.
