Based on record review, the licensee did not comply with the section cited above in when doing file reviews and observed the licensee's mandated reporter is currently expired as of 05/2024 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: By close of business on 10/16/2025 the Licensee will submit proof to LPA with an updated mandated reporter certification. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until coorrected.

Once completed LPA will follow up with the licensee to ensure the Certification course was completed.Failure to correct will result in a $100 per day civil penalty until corrected.
