Upon arrival LPA was greeted by licensee and was giving access to the home. LPA observed one assistant and licensee supervising 6 children consenting of 2 infants and 4 preschool aged children. LPA set up in the dining room and toured the home with Licensee. LPA consulted with licensee- all individuals subject to criminal background review have obtained a criminal record clearance. Licensee confirmed that there are no new residents or employees since the last inspection. LPA observed proper ratio and capacity was being followed. Facility hours of operation are Monday through Friday from 6 AM to 6 PM. A health and safety evaluation was conducted in all areas accessible to children. Off-limit areas include: second floor, garage and laundry room. Licensee acknowledged that children may never enter these off-limit areas. LPA observed that the facility is free from vermin, organized, safe, and in good repair. LPA observed a functioning smoke detector, carbon monoxide detector, and a full 2A10BC fire extinguisher. The facility has adequate toys that appear to be safe for children to use. The licensee stated there are no weapons or poisons in the home. LPA observed a fireplace barricaded according to regulation. The backyard is fenced and there are no bodies of water. LPA reviewed four children’s files which were observed to be complete. Required postings and the children’s roster were observed. Licensee has 2 infants enrolled. Licensee has a current fire drill log. Licensee’s CPR/First Aid card expires 5/2026. Licensee’s Mandated Reporter certificate expires 5/28/26. Licensee understands that training's must be completed every two years. There are two types of deficiencies: Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care. Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care. PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency. Based on record review, the licensee did not comply with the section cited above in that the licensee's mandated reporter training certificate expired and licensee's assistant did not have a mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to email the mandated reporter training certificates to LPA L. Habtom by POC date, 4/20/2024 via email.

Based on observation, the licensee did not comply with the section cited above in that 3 pack and plays had blankets in them and one had a cover above the pack and play which poses/posed a potential health, safety or personal rights risk to persons in care. No children were in the pack and play during the inspection.

Resolution: Licensee removed all items from the pack and play during the inspection. Licensee was also provided with the safe sleep PIN.

Resolution: Licensee will always have an assistant present when operating over a small family childcare ratio. Licensee will not operate as a large family without an assistant.

Resolution: Licensee will not care for more than four infants at any given time. Licensee understands that additional children must be a mix of preschoolers and school aged.
