Resolution: Licensee shall place a gate for each of the three stairs. LPA shall return for a plan of correction inspection.
Based on interview, and record review, the licensee did not comply with the section cited above, Licensee has not comepleted the renewal of mandated reporter training, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall take mandated reporter training tonight 02/26/2026. Licensee shall email the certificate of completion by the plan of correction date. loraine.perez@dss.ca.gov.
