Based on observation, the licensee did not comply with the section cited above Licensee did not maintain the portable play yards free from all loose articles and objects which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee immediately removed all blankets and pillows from portable play yards, bringing the facility back into compliance. LPA reviewed Safe Sleep Regulation with Licensee and assistant. licensee stated she shall maintain the portable play yards free from loose articles moving forward. LPA shall return for a Plan of Correction Inspection.
Based on record review, the licensee did not comply with the section cited above, Licensee's assitant does not have a current mandated reporter certificate which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LIcensee shall email or send by postal mail a copy of assitant's mandated reporter certificate by plan of correction date. LPA provided contact information.

Resolution: Licensee placed cleaning compounds and knives out of the reach of children. POC was cleared during annual inspection.
