Based on staff record review which revealed L1 did not have a current AB1207 mandated reporter training. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee started mandated reporter training (MRT) and when completed, will submit a copy of the MRT certificate to jennifer.patel@dss.ca.gov by POC due date.

Based on children's records reviewed the Licensee was unable to furnish proof that 15 minutes checks were being conducted for C2, C4, a C5. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will complete the sleep checks and log every 15 minute for children 24 months and under- including C2, C4, and C5. Licensee will provided LPA Patel a copy of completed sleep logs for C2, C4, and C5 from 10/7-14/2025 to jennifer.patel@dss.ca.gov.

Based on record review, the licensee did not comply with the section cited above in which 4 children (C2 - C5) did not have immunizations documented on the PM 286, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will fill out a blue card (PM286) for C2 - C4 and will make sure to fill out the blue card for each child who enrolls. Proof will be submitted via email, mail or fax. Email: Melinda.mohr@dss.ca.gov

Based on interview and observation, Licensee did not ensure Adult 1 (A1) had a Criminal Record Clearance, which poses an immediate risk to the Health, Safety and Personal rights of the children in care.
