Based on interview and record review, the licensee did not comply with the section cited above. 1 out of 3 infants did not have a complete sleep log.This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will ensure staff will complete sleep logs for all infants and will submit 3 weeks worth of sleep logs to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above. Licensee does not have a rescue pole. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will purchse a rescue pole with a body hook with a minimum fixed length of 12 feet and will submit proof to LPA by POC due date.

Based on interview and record review, the licensee did not comply with the section cited above. 1 out of 3 staff does not have a mandated reporter certificate on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will have staff complete training and will submit certifcate to LPA by POC due date.

Based on interview and record review, the licensee did not comply with the section cited above. Licensee does not maintain a daily inspection log. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will create a dail log inspection and will submit proof to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above. There is a bedroom window with access to the pool that does not have a barrier/fence outside of the window. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will put a barrier/fence outside of the bedroom window and will submit proof to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above. Pool gate door is not self closing, when licensee opened gate door, it remained opened. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will reinforce door to ensure self closing/self latching and will submit proof to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above. Pool gate door gap from ground to bottom of the door is greater then 2 inches. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will ensure the vertical clearance is 2 inches or less and will submit proof to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above. Life ring is not approved by US Coast Guard. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will purchse a Life ring approved by US Coast Guard and will submit proof to LPA by POC due date.

Based on record review, the licensee did not comply with the section cited above. Staff #2 and staff #3 did not have measles vaccine records in their file. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated staff #1 and staff #2 will get the measles vaccine and send a copy to LPA by POC due date,

Based on observation, interview, and record review, the licensee did not comply with the section cited above. During physical plant inspection, LPA observed licensee caring for children in garage that was converted in ADU and did not obtain fire clearance approval. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will relocate primary childcare area back into the living room that was previously approved by the fire department until fire clearance is approved for primary child care to take place in ADU.
