Based on observation, the licensee did not comply with the section cited above. Pool gate door mesh fencing was greater than 2 inches from the ground to the bottom of the fence. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will fix the fence door to close the bottom gap and will send proof to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above. One side of the mesh fence that is near a wall appeared to be loose which results there to be a gap between the mesh fence and the wall. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will fix the mesh fence to make it sturdy and will submit proof to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above. Facility did not have a life ring with a minimum exterior diameter of 17 inches and approved by the United States Coast Guard. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will purchase appropiate life ring and will submit proof to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above. Facility did not have a rescue pole with a body hook that had a minimum fixed length of 12 feet. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will purchase a rescue pole with fixed length of a minimum of 12 feet and will submit proof to LPA by POC due date.

Based on record review, the licensee did not comply with the section cited above. Staff #2 did not have a record of measles vaccine. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will submit measles vaccine for staff #2 to LPA by POC due date.

Based on interview and record review, the licensee did not comply with the section cited above which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will review the requirements for Mandated Reporter training, and renew the Mandated Reporter training and will submit proof to LPA by 10/26/2021.
