Based on record review, the licensee did not comply with the section cited above. Assistant did not have proof of immunization against pertussis and measles. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee Nubia Navarro stated she will have assistant receive vaccines or complete a antibody titer test to prove immunity and will send a copy to LPA by POC due date.
Based on observation, interview, record review, at 2:00pm, during staff file review both licensees mandated reporter training certificate were expired. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensees stated they will go online at www.mandatedreporterca.com to renew their mandated reporter training. Licensee will submit a copy of the certificate to licensing department by the due date.
