Based on observation, interview, record review, At 12:30pm, during staff file review assistant’s TDap vaccine was not available for review. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will send assistant to request a copy of their vaccine card to keep a copy in the assistant's file ready for review. Licensee will send a copy of the vaccine to licensing department by the due date.
