Based on facility Lead sampling tests it was discovered that Outlets A had high level of lead. This poses a potential risk to the health of children in care.

Designated staff was missing qualifying transcripts.

Based on observation, and record review, At 10:00am during staff file reviews, 3 out 4 files were missing immunization records for pertussis and measles. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

(a)(8) Personal Rights. Each child shall have the right to receive or reject medical care, or health-related services, except for minors for whom a guardian, conservator or other legal authority has been appointed.This requirement was not met as evidence by: Based on LPA's interviews, and record review, S1 gave C1 cashew which C1 was known to be allergic to and staff failed to administer EpiPen to C1 immediately as directed by C1’s parent & physician. Staff also failed to seek outside medical treatment immediately to help C1. This poses an immediate risk to the child's health and safety.
