Based on observation, interviews, and record review, the licensees did not comply with the section cited above in 2 out of 2 staff did not have a current Mandated Reporter training which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensees will email new mandated reporter training certificate to LPA via email at: dianna.valdezsantana@dss.ca.gov
Based on observation, interviews, and record review, the licensees did not comply with the section cited above in 2 of 2 staff did not have a current CPR certification which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Both licensees will renew their CPR certification and send LPA proof of current certification via email at: dianna.valdezsantana@dss.ca.gov
Based on observation, interviews and record review, the licensees did not comply with the section cited above in the twoe enrolled infants in care, licensees were not able to provide 15 minute check log documentation out of which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensees stated they are constantly observing the infants while napping but did not always document. LPA provided a sleeping log template they can use effective 4/11/23. Licensee will send LPA via email one month's worth of sleeping log documentation to send to LPA by 5/11/23/
Based on observation, interview, and record review, the licensee has not condcuted a fire drill in the last 6 months. Licensees did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated, they will conduct a frire drill on 2/8/22 and send a copy to licensing office by the due date.
Based on observation, interview, and record review, the licensee has not condcuted a fire drill in the last 6 months nor had documented the fire drill. Licensees did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated, they will conduct a fire drill and document on the fire drill log and send a copy licensing office.
