Based on LPM and LPA’s observation and staff interview. During the inspection on 03/08/2021, LPM and LPA observed 2 staff members supervised 3 infants and 1 preschool child in the infant classroom; 1 staff member supervised 9 school-age children which exceeded the term and condition granted for this license. This license allows facility to serve 8 school-age children only. Interviewed staffs confirmed that they sometimes co-mingled infants and preschool children in the morning for 10-15 mins so staff members can take a break to use the restroom. This poses a potential risk to the safety of the children in care.
