Based on observation, the licensee did not comply with the section cited above in LPA observed the master bedroom that is an off-limit area being used for a nap area which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will either submit an application to the license office for the master bedroom to get licensed or will submit a written statement stating that she understands she cannot use the master bedroom unless it's licensed. She will provide proof by the POC due date to LPA Nunez.
Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 2 files reveiwed adult providing care to children did not have Child Mandate Reporter training Certificate to review. Licensee stated adult has not taken training which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LIcensee stated they will send a copy of Child Mandate Certificate to LPA's email.
