Based on observation, the licensee did not comply with the section cited above. During facility inspection, LPA observed the carbon monoxide was not working. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: During reviewing the report with licensee, Licensee stated that her carbon monoxide did work and it was most likely the batteries that weren't working. LPA observed the licensee change the batteries. LPA tested the carbon monoxide and it is working. This deficiency was corrected during visit. LPA informed licensee it is important to check the batteries on carbon monoxide to ensure it is functioning.
Based on record review, the licensee did not comply with the section cited above. Licensee did not have a mandated reporter training on file. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will send proof of mandated reporter training certificate to LPA via email or text message by POC due date.
