Based on observations, the licensee did not comply with the section cited above in two out of three play yards, which poses an immediate health, safety or personal rights risk to persons in care. The LPA observed the play yards contained loose articles. One child was sleeping in one of the play yards. An infant was taken out of a car seat and placed inside a play yard with a pillow and a blanket.
Resolution: The licensee removed the loose articles while the LPA was still in the facility.
Based on observation, the licensee did not comply with the section cited above in one out of two infants in care, which poses an immediate health, safety or personal rights risk to persons in care. Upon arrival, the LPA observed an infant in a car seat. The car seat with the infant was in the living room with a school age child. The licensee stated the infant's mother had just dropped off the infant.
Resolution: The licensee removed the infant from the car seat and placed the infant inside a play yard.
Based on observation and records review, the licensee did not comply with the section cited above in two out of two infants. which poses an immediate health, safety or personal rights risk to persons in care. The licensee stated she could not find the logs she had completed for the infants. The licensee had logs for another child that is not an infant anymore.
Resolution: The licensee agrees to physically check on the infant every 15 minutes and log it as required by regulations. The licensee will provide proof of correction to the LPA via email by the POC due date above.
Based on observation and records review, the licensee did not comply with the section cited above in one out of four adults living in the home, which poses an immediate health, safety or personal rights risk to persons in care. The licensee stated that she provided evidence of clearance to the Department and stated that Elvia Garcia was cleared to be in the home according to another LPA. The LPA reviewed Guardian and the application showed as pending.
Resolution: The licensee stated that she will ask Elvia Garcia to temporarily stay with her sister and ensure that Elvia receives clearance before returning to the home.
Based on observation and records review, the licensee did not comply with the section cited above in four out of four children in care, which poses/posed a potential health, safety or personal rights risk to persons in care. The licensee had the names of two different sets of siblings in a single emergency card LIC700.
Resolution: The licensee stated she will provide the necessary documents to the parents so that each child has a complete, separate LIC700 in their individual separate files.

Resolution: The licensee stated she will provide the documents to the parents, have them sign and keep the bottom section of the LIC995A in the children's files.

Resolution: The licensee stated she will provide the documents to the parents so they can sign/date the forms and then place them in the children's files. The licensee will provide proof of correction by the due date via email.
