Based on observation the licensee did not comply with the section cited above. On 5/26/2023, LPA Lucero observed the window screen ripped, this poses a potential health, safety or personal rights risk to persons in care.
Based on obersrvation and interviews with 3 staff, the licensee did not... comply with the section cited above. Teachers dislcosed they are supervising more then 12 preschool children at one time for about 5-10 minutes. LPA observed on video footage, S1 alone with 22 children for 5 minutes. This poses an immediate risk to the health and safety of the children in care.
Based on observation the licensee did not comply with the section cited above. LPA observed an exposed water outlet that has sharp metal and is accessible to the children. This poses a potential health or safety risk to persons in care.

Limitations on Capacity and Ambulatory Status (a) A licensee shall not operate a childcare center beyond the conditions and limitations specified on the license, including the capacity limitation. This requirement was not met evidenced by: Based on interviews and record reviews, facility enrolled C7 who was under the age of two (infant) in the Early Preschool classroom. An infant was enrolled at 23 months old; this poses a potential risk to the health and safety of children in care.
