Based on observation, interview, and record review, the licensee did not comply with the section cited above in 2 out 3 staff did not complete the AB1207 Mandated Reporter Training and 1 out 3 did not have the training completed which poses a potential health, safety or personal rights risk to persons in care.

This requirement is not met evidenced by record review of the lab report analysis from Outlet B taken on 12/20/2022. Results indicated levels of lead in exceedance. This poses a potential risk to the health and safety of children in care.
