Based on observation and record review, the licensee did not comply with the section cited above. 2 out of 2 infants who were sleeping had blankets in their crib. This poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee immediately removed blankets from crib. Licensee stated she will no longer place blankets in the cribs and if the children are cold, she will turn up the heater.
Based on observation and interviews, the licensee did not comply with the section cited above. Child #1 was sleeping in the stroller for approximately 20 minutes. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated infants that fall asleep in strollers will be moved to a crib/play yard immediately.
Based on interview and record review, the licensee did not comply with the section cited above. 2 out of 3 staff did not have immunization records in their file. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will send copies of immunization records for staff to LPA by POC due date.

Resolution: Licensee stated she will send a copy of the LIC 9227 for 2 out of 4 infants who are currently under 1 year old and from now on will provide the LIC 9227 form for infants under 1 year old during enrollment.

Resolution: Licensee stated she will document a sleep log for each infant and provide a copy to LPA via email by POC due date.
