Based on observation, interview and record review, the licensee did not comply with the section cited above. The pool fence brand is Sentry Safe and the website did not indicate American Society for Testing and Materials (ASTM) International Standard F2286 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will contact Sentry Safe to make sure mesh is ASTM standard. Otherwise, licensee stated they will replace the mesh fence to be ASTM International Standard F2286. Licensee will email LPA of the information gathered from the company.

Based on observation, interview and record review, the licensee did not comply with the section cited above. The pool life ring is not United States Coast Guard approved. The Pool Life Ring does not have a label of United Stated Coast Guard USCG and no documentation shown to LPA which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will purchase a Life Ring that is US Coast Guard approved. Licensee will provide proof of US Coast Guard Life Ring to LPA

Based on observation and inspection, the licensee did not comply with the section cited above, the in-ground pool gate has a key and lock on top; however, the gate is not self-latching and the latch is 16” from the top of the fence which is 10" difference from regulation, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will fix the gate to be self latching and will show proof to LPA by due date of 11/04/24

Based on interview and record review, the licensee did not comply with the section cited above, the facility did not conduct disaster drill in the last 6 months, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will conduct an emergency drill and will send proof to LPA by due date of 12/2/24

Based on interview and record review, the licensee did not comply with the section cited above , 1 child is missing immunization which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated a proof of immunization for child will be provided to LPA by due date of 12/2/24

Based on record review, the licensee did not comply with the section cited above 2 infants does not have current 15-minute safe sleep log which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will proved LPA a copy of 15-minute infant safe sleep log by due date of 12/2/24

Based on observation, the licensee did not comply with the section cited above. Playpens had loose blankets and pillows while children were sleeping. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will not have blankets or pillows in the play pens for infants.

Based on observation and record review, the licensee did not comply with the section cited above in. LPA observed 13 children in care, 6 of which are infants.This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will create a staggered schedule and send a copy to LPA via email by POC due date.
