Based on observation, interview and record review, the licensee did not comply with the section cited above in 3 out 4 children present did not have a copy of their immunization record in their file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send LPA proof of Child #1, Child #2 and Child #4's proof of immunizations by POC due date.

Based on observation, interview and record review, the licensee did not comply with the section cited above in 1 out 1 enrolled infant did not have proof of 15-min check log in the child's file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send LPA two weeks worth of 15-minute checks by POC due date. Licensee stated she does check the infant every 15-minutes but was not documenting it and she understands she must always document the sleep check for infants 0-24 months of age.

Based on observation, interview and record review, the licensee did not comply with the section cited above in 2 out of 2 infants did not recieive the LIC9227 form upon enrollment therefore the forms were missing in their 2 files which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will have the parents of the two infants complete the form even though they are now over 1 yr. old. Licensee will email or send LPA the completed forms and she understands that she must provide the LIC9227 upon enrollment for any children 0-12months of age. LPA provided licensee the LIC9227 form.
