Based on observation, the licensee did not comply with the section cited above, one count of infant C6 sleeping in a pack and play with blankets and a bottle which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will review safe sleep regulations provided by LPA Sims and send detailed statement of Licensee's understanding of regualtions and agreement to follow the regulations. Licensee will have all parents with children enrolled sign LIC 9224 and send copy of LIC 9224 to LPA Sims by 02/06/26

Based on observation, the licensee did not comply with the section cited above in one count of Licensee having infant C1 sleeping on the couch which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will review safe sleep regulations provided by LPA Sims and write statement that the Licensee will not allow infant to sleep in equipment not approved by community care licensing. Licensee will have all parents with children enrolled sign LIC 9224 and send copy of LIC 9224 to LPA Sims by 02/06/26

Based on observation and record review, the licensee did not comply with the section cited above, Licensee has not docuemnted a fire drill since 04/10/25 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a fire drill by 02/19/26 and document the fire drill on the fire drill log. Licensee will send copy of the fire drill log to LPA Sims via email by 02/19/26 to sydney.sims@dss.ca.gov

Based on record review, the licensee did not comply with the section cited above in one count of infant C6 not having a file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain all documents required for C6's file and send copy of the paperwork to LPA Sims by 02/19/26 via email to sydney.sims@dss.ca.gov

Based on observation, the licensee did not comply with the section cited above, Licensee has 9 children present with no assistant which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will review ratio and capacity regualtions provided by LPA Sims and send detailed statement to LPA SIms stating that the Licensee understand the regualtions and agress to follow the regulations by 02/19/26 via email to sydney.sims @dss.ca.gov

Based on record review, the licensee did not comply with the section cited above , which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct 1 weeks worth of safe sleep checks for all infants in care and send copy of log to LPA Sims via email to sydney.sims@dss.ca.gov by 1/28/25

Based on observation, the licensee did not comply with the section cited above, Licensee was not using proper sheet for pack and plays where infants were sleeping which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will watch safe sleep video provided and sent statement to LPA Sims verifying that Licensee watched and udnerstand the video and agrees to follow the CCL's infant safe sleep regulations. Licensee will send statement to LPA Sims via email to sydney.sims@dss.ca.gov by 2/14/25

Based on record review, the licensee did not comply with the section cited above, Assistant has expired mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licnesee will have assistant complete mandated reporter training and send copy of certificate to LPA Sims via email to sydney.sims@dss.ca.gov by 2/14/25

Based on observation, the licensee did not comply with the section cited above in wall heater gate not secured to the wall which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to get a gate to keep the heater innaccessible to children in care. Licensee agrees to send a photo to LPA by 4/10/2024 @ tammy.dutra@dss.ca.gov.

Based on record review, the licensee did not comply with the section cited above in 2 children in care missing signed copies of LIC 627, LIC 995 for C1, Immunization record for C6, Sleep Plan for C6 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to send copies of missing signed documents to CCLD by 4/10/2024. Licensee to email documents to LPA @ tammy.dutra@dss.ca.gov.

Based on record review, the licensee did not comply with the section cited above in two out of two infants files do not contain 15 minute checks which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to email 3 days worth of checks for two infants in care to CCLD by 4/10/2024. Licensee agrees to email the documents to tammy.dutra@dss.ca.gov.
