Based on observation, interview, record review, the licensee did not comply with the section cited above in 1 of 1 staff did not have current childcare mandated reporter training certificate for LPA to review, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will complete childcare mandated reporter training and provide proof of certificate to LPA by POC date of 03/09/26
Based on observation, interview, and record review, the licensee did not comply with the section cited above in 1 of 4 children do not have 15 minute sleep log documented, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will start documenting safe sleep checks for infants newborns - 2 years old every 15 minutes and note labor of breathing, and other observations. Licensee will send LPA sleep log for infants in care on POC date 03/06/2026

Resolution: Licensee will provide proof that they were immunized against influenza (or a declenation stating they are declining the influenza vaccine), pertussis, measles and TB by POC date.
Based on observation, interview, record review, the licensee did not comply with the section cited above in 1 out of 2 children did not have Individual Sleep Plan (LIC9227) in file for LPA to observe, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure Individual Sleep Plan (LIC9227) is completed by the parent of C2 and send proof to LPA by POC date.
Based on observation, interview and record review, the licensee did not comply with the section cited above in 1 out o2 children did not have Notification of Parents rights (LIC 995), Affidavit Regarding Liability insurance (LIC 282) and Consent of Emergency Medical Treatment (LIC 627) in file to LPA to observe which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure Notification of Parents rights (LIC 995), Affidavit Regarding Liability insurance (LIC 282) and Consent of Emergency Medical Treatment (LIC 627) are completed by the parent of C1 and send proof to LPA by POC date.
