Upon arrival LPA Lee met with Director Zhaoyuan Huang. The purpose of the inspection conducted on tis date was to obtain additional information regarding an incident report provided by the facility on 07/08/2024. On 07/08/2024 Child#1 sustained an injury at the facility when the classroom was using the restroom. According to the facility Staff#1 was inside the restroom with 11 children including Child#1. Staff#1 stated that they were in the middle of changing a child on the changing table that was located near the restroom sink, when Child#1 walked out of the restroom. Staff#1 told Child#1 to come back inside the restroom. Child#1 was walking back in to the restroom when Child#2 closed the restroom door while Child#1's hand was near the door area. Child#1 injured their hand when the restroom door was closed by Child#2. While Staff#1 is qualified to provide care for 12 preschool children in the classroom, the same type of supervision was not possible since Staff#1 was changing another child while the other remaining 10 children were inside the restroom. The facility stated that normally another staff member would be present to help out Staff#1 when going to the restroom, but there was no one available at that point in time on 07/08/2024. The fact that Staff#1 was inside the restroom with 10 children while changing a child is an immediate risk to children in care. Please see the attached 809D for the specific deficiency information. The notice of site inspection and this report must remain posted for a period of 30 days during hours of operation. Failure to maintain posting during business hours will result in a civil penalty of $100.00 dollars. A copy of the LIC 9224 was provided to the facility during this inspection. The Director understands that a signature on the copies of this form is to be filed for every child currently attending and any new children enrolled for a period of 1 year starting from this date.
