Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in [count] out of [total count] [(objects) (persons)] [identifiers] which poses/posed a potential health, safety or personal rights risk to persons in care. Upon review of records it was revealed that the licensee was unable to locate her assistants' mandated reporter certificates and licensee's certificate is expired.
Resolution: Licensee and assistants will update their mandated reporter training and print updated certificates to be put into there files by 11/7/25

Upon review of records it was revealed that both assistants do not have immunization records in file
Resolution: Licensee will have each assistant obtain their immunization records to be put into their files. Records must show proof of MMR, PERTUSSIS, FLU. TB test results. (mmr, dtap,flu) or a letter declining the flu vaccine.
