Based on record review, the licensee did not comply with the section cited above as two assistants did not have proof of immunization for MMR on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall submit to CCL proof of immunization for MMR for both assistants and place a copy in their file by 10/13/2025. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.
Based on record review, the licensee did not comply with the section cited above one child did not have immunization record in file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee was able to have parent email a copy of the child's immunization record prior to LPA leaving the facility. The POC is cleared as of 9/19/2025.
