Based on record review, the licensee did not comply with the section cited above as fire/disaster drills have not been conducted in the past 6 months which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall conduct and log a drill and submit it to CCL by 10/16/2025. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Based on record review, the licensee did not comply with the section cited above as the licensee's mandated reporter certificate is expired which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall submit a copy of her mandated reporter certificate to CCL by 10/16/2025. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.
