SZELOG FAMILY CHILD CARE

Based on interview and record review, the licensee did not comply with the section cited above in 3 out of 6 records reviewed, they did not contain immunization records for the children enrolled which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will request records from parents and send a picture of the immunization records pending for 3 children by the date noted above. Copy of the records can be send via text or email.

Based on observation, , the licensee did not comply with the section cited above the FCCH has not purchased a fire extinguisher since 2022, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will purchase a fire extinguisher and will send a picture to LPA by the date above indicating they have installed in the play room, picture can be sent via email or text, LPA provided business card.

Based onobservations, the licensee did not comply with the section cited above in there was essential oils observed around the sink in the playroom, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will remove all items that are considered hazardous to children, Licensee willl send picture to LPA displaying the area will be free of any item that can pose a danger to the children in care by the date above.

Based on interview and record review, the licensee did not comply with the section cited above they have not completed a disaster/fire/ earthquake drill in the last past months which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a drill and log it on a drill log and send a picture of the drill log complete via email or text by the date indicated above.

Based on observation , the licensee did not comply with the section cited above as they do not have a functioning caborn monoxide detextor which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will purchase a carbon monoxide detector and will send a picture to LPA by the date above indicating they have installed in the play room, picture can be sent via email or text, LPA provided business card.

Based on observation, interview, and record review the licensee did not comply with the section cited above. Licensee failed to maintain a fire/disaster drill log at least once every six months. Failure to document or conduct the drills poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Provide LPA Thompson documentation showing fire and disaster drills are being conducted every six months by plan of correction due date via email or text.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee and assistant CPR/First Aid certification both had an expiration date of 10/6/2021, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Provide LPA Thompson proof of CPR/First Aid certification by plan of correction due date via email or text. CPR/First Aid certification must show pediatric.

Based on observation, interview, and record review, the licensee did not comply with the section cited above for family child care home. Licensee failed to purchase or service a 2A10BC fire extinguisher once a year which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Provide Proof of purchase or service date by March 31,2022 via email or text to LPA D. Thompson.
